CHEMICAL STABILITY AND COMPATIBILITY OF DOBUTAMINE, DOPAMINE, AND NITROGLYCERIN IN TOTAL PARENTERAL NUTRIENT SOLUTION

Abstract
IPA COPYRIGHT: ASHP The stability of dobutamine, dopamine, and nitroglycerin in total parenteral nutrient (TPN) solutions was studied. Two concentrations of each drug (dobutamine: 250 and 1000 ug/mL; dopamine: 400 and 3200 ug/mL; and nitroglycerin: 100 and 400 ug/mL) were separately added to three different TPN solutions. The TPN solutions consisted of electrolytes and 10%, 25% or 35% dextrose with 2.5%, 4.25% or 5.0% amino acids, respectively. All Solutions were assayed immediately after mixing (time 0), and then 1, 2, 4, 8, 24 and 48 hours later using high-performance liquid chromatography. The concentration of each drug and the 14 individual amino acids was measured and percent recovery was determined. Dobutamine exhibited stability (|GT/ 90% recovery) in TPN solution for 48 hours. Dopamine showed variable stability in TPN solution, with the drug being the least stable in the more highly concentrated amino acid-dextrose solutions. Nitroglycerin stability was evident in all TPN solutions for at least 4 hours, with two of the admixtures remaining stable up to 8 hours. All amino acids assayed were stable for at least 1 hour in the dobutamine-TPN admixture. Glycine, histidine, lysine and threonine were the only amino acids to fall below 90% recovery after 1 hour, and the remaining amino acids were stable up to 48 hours. Two amino acids (glycine and lysine) were stable for less than hour in dopamine. Four other amino acids (alanine, leucine, threonine and tyrosine) broke down in nitroglycerin in less than 1 hour. This study concludes that dobutamine may be administered with TPN solution of various concentrations using the piggyback method. Nitroglycerin may also be administered with TPN without altering the stability of the drug. However the instability of some amino acids in nitroglycerin should be considered. The use of common lines for dopamine and TPN solution is not recommended.
