IPA COPYRIGHT: ASHP A study evaluating the compatibility of 12 drugs commonly used in critical care when combined in 35 different combinations diluted with 5% dextrose, combined under simulated Y-site conditions, and observed for 24 h at room temperature is reported. Five combinations were incompatible in a concentration-dependent fashion. Human insulin (Humulin R) and labetalol hydrochloride were inconsistently compatible at 1 unit/ml of insulin and up to 5 mg/ml of labetalol. Insulin was compatible with <3 mg/ml of dopamine hydrochloride. Norepinephrine bitartrate and insulin developed a precipitate when norepinephrine exceeded 0.05 mg/ml; lower concentrations appeared to be compatible with higher insulin concentrations. Heparin sodium was incompatible with labetalol at concentrations of 1-2.5 mg/ml and heparin concentrations of as low as 4 units/ml. Heparin was also incompatible with dobutamine hydrochloride at dobutamine concentrations of 1-2 mg/ml, and with 1.6-3.25 mg/ml of nitroglycerin.
