IPA COPYRIGHT: ASHP The clinical relevance of silent asymptomatic peptic ulcer disease and associated life-threatening complications is discussed. It is estimated that 5-10% of peptic ulcers remain asymptomatic throughout life. Silent ulcers are more frequent in the elderly and those treated with nonsteroidal anti-inflammatory drugs. Death and perforations are more frequently due to silent than symptomatic ulcers. Studies have shown that maintenance treatment with H2-antagonists, at established doses, does not strictly control ulcer recurrences. Subpopulations at risk should be examined for a silent peptic ulcer and treated accordingly.
