IPA COPYRIGHT: ASHP Reported is a case of propylene glycol (I) intoxication in a 58-yr-old schizophrenic man with renal disease, accompanied by the development of lactic acidosis and I levels of 70 mg/dl in the blood and 60 mg/dl in the urine. The patient quickly responded to IV fluids and sodium bicarbonate. Lactic acidosis may have developed as a result of a large excess of I that was metabolized to lactic acid. Since the patient had azotemic renal disease, I probably accumulated in the blood because of impaired renal clearance secondary to renal disease.
