IPA COPYRIGHT: ASHP The case of a 21-yr-old male who developed aplastic anemia after applying lindane (Kwell) lotion from his neck to his knees twice daily for 3 wk for presumed scabies is reported. A blood lindane level of 20.7 ng/ml was reported. The patient was empirically treated with systemic antibiotics and a 10 day course of antithymocyte globulin (ATGAM) and 20 mg of prednisone twice daily. Liver function tests returned to normal and the patient was released. Blood lindane levels decreased over the next several months to 0.5 ng/ml. Follow-up as an outpatient noted little change in blood cell counts. A second and third course of antithymocyte globulin had no effect. Thirty-six months after presentation, the patient was receiving no medication and had marked improvement in white and red blood cell counts with a modest improvement in platelet counts.
