An operated case of left renal vein entrapment syndrome

We report here a case of left renal vein entrapment syndrome managed by relieving the venous compression using ring-reinforced polytetrafluoroethylene prosthesis. The patient was a 22 year-old man who complained of hematuria and left flank pain. MRI revealed the left renal vein, which became narrow between the aorta and the superior mesenteric artery. Selective renal angiography revealed the left renal vein compressed between the aorta and the superior mesenteric artery. We diagnosed left renal vein entrapment syndrome (the so-called nutcracker phenomenon). At operation, to relieve the compression, a 16mm ring-reinforced polytetrafluoro-ethylene prosthesis was selected to create an external stent around the left renal vein. Postoperative recovery was uneventful.Hematuria and left flank pain gradually subsided. We recommend this simple and physiologic procedure to the nutcracker syndrome.
