A 47-year-old man experienced ventricular fibrillation in the early morning. A 12-lead electrocardiogram obtained immediately after resuscitation showed no significant ST-T changes. An emergent coronary angiogram also showed no significant lesion in the coronary arteries. However, the patient developed ST elevation that led to ventricular fibrillation during induced mild therapeutic hypothermia. Immediate defibrillation followed by isosorbide dinitrate administration resolved the ST elevation.
