The romanian health system is characterized by lack of transparency, poor funding, insufficient numbers of medical personnel, outdated equipment, and negligence. These risk factors are sometimes manifesting together, giving rise to correlated risks that appear in cascade, causing major losses, both financial and human. Unfortunately, most managers of health institutions in Romania show a lack of management skills and reaction to increasing risk exposure of the institutions they lead. This is explained by the fact that they are either politically appointted, or are physicians who are not specialized in hospital risk management. In this paper we present a case study describing a tragedy that happened at an important hospital in Romania, where the manifestation of several types of risks generated human and financial losses, and strongly affected the hospital reputation that has reached a critical point. How could this situation have been avoided or fixed? What are the ways of evaluating, monitoring and managing the similar risks? And could the risks be prevented in time?
