The use of medication by breastfeeding women implies that nursing infants may be exposed to the medication through ingestion of breast milk. We discuss safety in terms of the magnitude of the risk to the baby from this practice, and we emphasize that safety needs to be evaluated in each patient's case. To assist in this decision, this chapter summarizes determinants of drug excretion into milk and the methods used to estimate a nursing infant's dose or exposure. An extensive summary of data obtained from selected studies on drug excretion into milk is provided. Some conclusions are made regarding the safety of these medications during lactation. The health professional should not that we do not recommend the use of any drug without the advice that each nursing infant be monitored for any adverse effects during maternal drug therapy. The intensity of this monitoring will vary with the potential risk and with the mother's own concerns.
