Hypoglycemic and uricosuric properties of acetohexamide and hydroxyhexamide

Acetohexamide has a moderate uricosuric as well as a pronounced hypoglycemic effect, which makes it a useful drug in subjects with coexisting diabetes and gout. The hypoglycemic property is mainly due to its metabolic effect on the β cells on the pancreas, whereas the uricosuric property depends upon an effect on the renal tubules, namely inhibition of tubular reabsorption of uric acid. Dissociation of the dual properties is evident in subjects with renal insufficiency, in whom the hypoglycemic response is not accompanied by uricosuria. The uricosuric property of acetohexamide is most likely due chiefly to its metabolic product, the L enantiomorph of 1-hydroxyhexamide. No difference in uricosuric activity was found between the levo and the racemic forms of 1-hydroxyhexamide. 1968.
